Associa FHA Assist - Program Agreement
CERTIFICATION ASSISTANCE PROGRAM AGREEMENT
Although it is understood that your management company is not responsible for ensuring that Association is FHA
certified, due to the upcoming changes to the FHA underwriting guidelines, your management company will assist
Association in becoming FHA certified.
It is understood by your management company that Association would like to proceed with an application for FHA
certification.
In accordance with this process, I,________________________________________, an officer of the above
named Association, (signature below) hereby authorize my management company to work with Associa FHA
Assist by preparing necessary information to apply for FHA certification and to charge the following fee:
A deposit of $350 due upon execution of the agreement (fee will be invoiced);
Payment of $650 upon approval of the condominium association by FHA
(fee will be invoiced for).
I understand and grant Associa FHA Assist permission to use the association data to submit an application to the
Federal Housing Administration (FHA) and to discuss the association's application with FHA officials.

* This fee includes assembly and submission of an FHA application; however, in some instances, the FHA requires attorney review of certification of an application.
The fees for the application do not include any attorney fees or costs that may arise in the process. If an attorney certification is required, Associa FHA Assist will
notify the client and provide options at that time.
